Claude Code 2.1.193: auto-mode classify all shell, file autocomplete, OTel logging

Version 2.1.193 adds auto-mode setting to route all shell commands through classifier, live file path autocomplete in bash mode, and OpenTelemetry logging for assistant responses. Also includes memory-pressure reaping for idle background commands and various bug fixes.
