Claude Code 2.1.196: org defaults, AWS gateway, agent hooks

Version 2.1.196 adds organization default models (set by admins in org console) and background agent notifications that fire hooks on need input/completion. The built-in Explore agent now inherits the session's model (capped at opus) instead of running on haiku, and transient network errors now retry with backoff.
