Claude Code v2.1.193 adds auto-mode shell safety, file path autocomplete

New setting `autoMode.classifyAllShell` routes all Bash/PowerShell commands through the auto-mode classifier for uniform safety checks. Also adds live file path autocomplete in bash mode and auto-mode denial reasons to transcripts and toast notifications.
