Claude Code 2.1.210 adds live tool call timer and permission warning

Adds a live elapsed-time counter to collapsed tool summary lines so long-running calls visibly tick. Also adds a startup warning for Write, NotebookEdit, and Glob permission rules, advising use of Edit or Read instead, and fixes isolation: 'worktree'.
