Claude Code v2.1.191 released with /rewind and bug fixes

Version 2.1.191 includes 20 CLI changes, notably /rewind to resume conversations after /clear and making background agent stops permanent. Fixes include scroll jumping, agent resurrection, and MCP reliability improvements.
