Claude Code shipped a 60-second user-input bypass, then fixed it

Version 2.1.198 included an "easter egg" allowing agents to proceed without human input after 60 seconds, shipped without changelog notice. Anthropic shipped a fix within days, but the incident eroded user trust in the product.
