Claude Code 2.1.193: auto-mode shell classifier, OTEL logs, autocomplete

Version 2.1.193 adds a `autoMode.classifyAllShell` setting to route all shell commands through the auto-mode classifier. It also adds OpenTelemetry logging for assistant responses and live file path autocomplete in bash mode.
