Claude Code 2.1.196: org default models, background agent auto-PR

Adds support for organization default models set in the admin console, and background agents now auto-commit, push, and open a draft PR when completing code work. Also introduces /datavizskill for chart guidance and adds Claude Platform on AWS as a Gateway provider. Fixes multiple bugs including network drop retries and stuck background tasks.
