Claude Code 2.1.204 fixes hook streaming, memory, and agent bugs

Fixes hook events not streaming during SessionStart hooks, a memory and CPU regression in interactive sessions, and background agents becoming unresponsive due to stale tokens. Also adds warnings for expiring logins and shows permission mode badge.
