Claude Code v2.1.195 adds env var to disable mouse clicks

Adds CLAUDE_CODE_DISABLE_MOUSE_CLICKS to disable mouse interactions in fullscreen mode while preserving scroll. Fixes hook matchers to use exact-match instead of substring matching for hyphenated identifiers like 'code-reviewer'.
