Claude Code 2.1.203: background session recovery, security fixes

Fixes background sessions becoming unresponsive when daemon token goes stale, and blocks unauthorized destructive security actions while allowing CTFs/tests with approval. Also fixes bash 'argument list too long' errors in repos with many git worktrees.
