Claude Code v2.1.191 adds /rewind and fixes background agent resurrection

Claude Code v2.1.191 includes 20 CLI changes, with a new /rewind command to resume conversations from before /clear. Background agents stopped from the tasks panel are now permanently terminated. MCP capability discovery and OAuth retries improve reliability.
