Claude Code 2.1.203: login warning, background agent fixes

Version 2.1.203 adds a login expiry warning and shows a manual permission mode badge. It fixes background agents permanently unresponsive when daemon token goes stale and a macOS low-memory stall regression. The update also sends session working directories to MCP roots/list.
