Claude Code v2.1.191 adds /rewind, fixes MCP and agent issues

Claude Code v2.1.191 adds /rewind to resume conversations after /clear and fixes background agents permanently stopping. MCP capability discovery now retries on transient errors, and sandbox network permissions are remembered per session.
