Claude Code 2.1.204 fixes hook streaming, memory regression, and auth expiry

Fixes include hook events not streaming during SessionStart hooks in headless sessions, a memory/per-turn CPU regression in interactive sessions, and a warning when login is about to expire. Also fixes background session recovery and PATH inheritance on Windows.
