Survey: Half of enterprises shipped AI agents that failed in production

Across 157 enterprises, half have shipped an AI agent that passed internal evaluations but failed a customer in production; only 1 in 20 fully trusts automated evals. Most are shipping to production despite the reality-alignment gap.
