Claude Code v2.1.193 adds auto-mode settings for shell commands

New autoMode.classifyAllShell setting routes all Bash/PowerShell commands through the auto-mode classifier. Also adds auto-mode denial reasons to transcript, denial toast, and /permissions recent denials.
