Claude Code v2.1.193 adds auto-mode shell classification

Adds autoMode.classifyAllShell setting to route all Bash/PowerShell commands through auto-mode classifier. Also includes auto-mode denial reasons in transcript, denial toast, and /permissions recent denials.
