Claude Code 2.1.193: auto-mode classifier, live path autocomplete, and more

Version 2.1.193 of Claude Code introduces the autoMode.classifyAllShell setting to route all shell commands through the auto-mode classifier, live file path autocomplete in bash mode, and automatic memory-pressure reaping for idle background shell commands. Also added: OpenTelemetry logging for assistant responses, denial reasons to transcript, and MCP auth auto-reconnect on 401/403.
