Claude Code 2.1.204 released with bug fixes

Version 2.1.204 fixes hook streaming in headless sessions, adds login expiry warnings, and improves background agent resilience. Also addresses a macOS stalling regression (from 2.1.196) and a memory/CPU regression in interactive sessions.
