Claude Code 2.1.204 fixes hook streaming, memory regression, and more

Version 2.1.204 fixes hook events not streaming during SessionStart hooks, adds login expiry warning, and includes background session improvements. It also fixes a memory regression where the context-usage indicator re-analyzed the entire transcript every turn.
